C. To apply for a benefit claim on your matured plan:

  1. Print and fill out the Benefit Settlement Form (BSF).
  2. Email the scanned copy of the fully filled out BSF to [email protected] together with the scanned copy of the following:
    • One (1) government-issued ID with 3 specimen signatures
    • Certificate of Full Payment/Plan Contract
    • Proof of bank account (e.g. validated deposit/withdrawal slip, SOA, or front-page copy of passbook)
    • Nominee’s proof of eligibility to enroll in College/High School (as per plan type). This is a mandatory requirement for Education plans except Prodigy, Magna, and Summa plans.
