How do I avail of the Lump Sum Benefit?
Please submit a photo or a scanned image of the following requirements to our email at [email protected].
- Benefits Settlement Form*
- Authority to Deposit Form*
- Plan Contract and Certificate of Full Payment
- One government-issued ID with three specimen signatures
- Bank passbook, bank statement, or validated deposit slip showing the bank account name and number
*Printed copies of these forms must be accomplished by PhilPlans planholders. Wet signatures are also required.
How will I get my Lump Sum benefit?
Your benefit will be deposited directly to your personal accounts in any of PhilPlans’ accredited banks.
What happens to my plan when I avail of the Lump Sum Benefit?
Your plan will be considered fully-availed after the release of your lump sum benefit.
